Periodontal Disease and Smokers

Periodontal disease is more prevalent and more severe in smokers than in nonsmokers. The host response in smokers is altered by a number of factors including increased gingival crevicular fluid, adhesion molecules and genetics. In addition, smokers are poor healers compared to nonsmokers and their response to periodontal therapy can be compromised which can result in continued progression of periodontal disease, or the need for more treatment.

The overall goal of this presentation is to provide the clinician with information on periodontal disease and its treatment in smokers. Following this presentation, the participant should be able to do the following:
  • List the categories of periodontal disease
  • List and describe the prevalence of periodontal disease in smokers and nonsmokers
  • List and describe the differences in the smoker’s host response in periodontal disease
  • List and describe the risk factors for peri-implantitis
  • List and describe the smoker’s response to periodontal treatment and possible treatment options

Presenter: Richard Nejat, D.D.S.
Dr. Richard Nejat is board certified by the American Board of Periodontology. His practice is on the forefront of computer guided implant dentistry and minimally invasive dental surgery. He is a course instructor in various continuing educational seminars, symposiums to colleges, and dental societies. Dr. Nejat attended Drew University and continued his education at New York University, where he attained his doctorate of dental surgery. Dr. Nejat was elected to membership in Omicron Kappa Upsilon, the National Dental Honor Society recognizing academic and clinical excellence in dentistry. He earned his certificate in periodontics from the State University of New York at Stony Brook. Dr Nejat is currently clinical assistant professor in the department of implant dentistry and periodontics at New York University and maintains private practices in Manhattan and Nutley, New Jersey.

Each of the following continuing education courses is offered via the ADHA and provides either 1 or 2 CE hours. Additional courses can be found on the site for  $15 (ADHA members) or $30 (non-members), with the exception of course noted below which are provided at no charge.

Cosmetics, health, function, and long-term prognosis all depend upon accurately assessing how a patient presents and then determining how to proceed. Individual patient considerations include oral hygiene, the presence of removable prostheses, the status and contouring of restorations, gingival anatomy, long-span fixed prostheses and caries. Attention should be given to the periodontal-restorative dynamic throughout all the stages of treatment from diagnosis and treatment planning to final insertion.

The overall goal of this course is to provide the participant with information on the restorative considerations and factors that can impact periodontal health. Upon completion of this course, the participant will:

•Know the restorative factors associated with periodontal disease.

•Know the importance of biologic width and considerations in the placement of restorative margins.

•Understand the importance of full treatment planning that takes into consideration appropriate restoration designs for periodontal health, and the appropriate sequence of treatment for a given patient.

Ineffective communication skills, rushed appointments and/or fear of what the patient may think when presenting a periodontal diagnosis are some of the common barriers that interfere with existing patients being enrolled into non-surgical treatment in the general dental practice. What do you say to a long term patient that has periodontal disease, but it unsuspecting? This course offers solutions for the entire team to create value for periodontal treatment; manage common patient objections; and move a patient from routine prophylaxis into necessary non-surgical treatment inclusive of locally applied antimicrobials, as indicated, and on-going periodontal maintenance.
